Care Management
Utilizing strong clinical expertise in a cost-effective staffing model, we provide care management services focusing on clinical guidelines adherence, care coordination, preventive care, and lifestyle influences with patients. Our care management services support greater patient compliance with the treatment plan, enhancement of patient enrollment rates, and improved billing efficiencies resulting in an extremely high-performing care management program for our provider clients.

When there are gaps in patient care, individuals are more likely to experience poor health outcomes, resulting in hospital readmissions, lower quality care poor patient satisfaction scores, and increased costs for healthcare providers. To address these challenges, we provide a focused approach that includes patient outreach, care coordination, pre-visit planning, and bridging the gap between clinical visits.

Management
The Care Intervention team is prepared to help patients with all types of chronic diseases. Many patients with chronic illnesses have multiple disease conditions or co-morbidities. Our approach to chronic disease management integrates physicians, clinical support teams, and patients within industry-recognized clinical guidelines to provide proven results. We strive to support our medical provider’s prescribed care plans by educating patients, taking into
consideration of many social determinant factors such as lifestyle, emotional health, sleep quality, and overall well-being.

Post-surgical
Follow-up
Post-surgical follow-up services are carried out immediately after the patient has a surgical procedure and typically extend for a few days until the patient is determined to recover or has minimal risk of complications. Our Care Interventions team follows up with the patient through telehealth or virtual visits ensuring the patient follows the prescribed recovery plan and is absent of complications. Our team also follows up with the patient’s direct care team to advise of potential complications or risks to the patient as a result of the surgical procedure.

FAQ's
Care Interventions overseas team members can provide multiple options for time zone. In instances where normal business hours are required, we will set those shifts for our team members to align with those hours. In instances where tasks, such as identification of gaps in care or quality gaps for contractual needs, do not require alignment with normal business hours we can assign our team members to different times that would align with the completion of tasks as your business hours begin.
Care Interventions only uses seasoned medical professionals from the Philippines. Those professionals are under our BAA and understand the requirements for policy, procedures, or clinical guidelines and will be trained to follow those requirements.
The Philippines prides itself on being an English-speaking country. Starting in primary school students are taught English as a second language. Care Interventions takes great pride in not only matching professionals with exceptional English skills but even matching specific locations in the US where English diversity requires a different enunciation of response.
Care Interventions will place medical professionals that will be a supplement to your own team. We encourage full communication with those professionals through a collaborative and cross-communicative manner. Care Interventions will also be providing reporting of contacts, patients spoken with, etc. so that we are providing a full array of communications back to your organization.
As we stated in the response to FAQ 4, we pride ourselves on placing medical professionals aligned with your own team. Your medical protocols and requirements will be followed, and Care Interventions will train our team members for your organization needs. It is our hope that you can treat our medical professionals as your own team members and work collaboratively
with them in all things.
Care interventions will staff medical professionals that meet your needs. If that is an RN that specializes in cardiovascular disease or a general RN, we can provide those skillsets. RNs in the Philippines all have their BS in Nursing and follow very strict educational requirements that are designed to replicate the US education system. Furthermore, many of our professionals
have a minimum of 2 years hands-on nursing experience in a hospital or clinic.
The care Interventions leadership team consists of medical professionals that have years of experience working within the US Healthcare System. That is why we have taken great care in creating a model where HIPAA compliance, when exposed to protected health information, is a top priority. Our offshore teams have signed the Care Interventions subcontractor BAA to ensure we have a full understanding of the protection necessary by our medical professionals when handling protected health information.
